WCVA is producing a web-based portfolio of third sector good practice within Health, Social Care and Wellbeing. Through this portfolio they aim to strengthen knowledge and understanding of third sector service provision among policy makers, service planners and providers and celebrate the ability of the third sector to provide effective citizen-centred services. They are now requesting examples of good practice within the third sector and hope that you might be able to participate. If your organisation or an organisation within your area has been particularly successful at delivering a service, collaborating with other organisations to produce a 'linked up' service, or provided added value they would like to hear from you. They are considering all examples within the field of Health, Social Care or Wellbeing services, however, the below categories can be used as a guide for ideas to put forward:

The response to the good practice portfolio among the third sector has already been very strong. Consequently, in order to fairly select among examples, it is asked that your organisation complete the attached preliminary questionnaire.
